If you have first-hand information or documents about uncollected receivables, duplicate payments or avoidable penalties at the Ministry of Health or another federal body, fill in the form on that page (name, email, type of fraud, description, optional attachments) or write to info@ofag.gov.et, the address printed on the OFAG contact page. Describe what happened, when, and which institution, and attach evidence you are entitled to share.
