Vermont prison left a detoxing man unchecked for hours until he was found dead
Jeffery Nichols, 35, was booked into Northwest State Correctional Facility in St. Albans on a domestic assault charge and told a nurse he was withdrawing from heroin and cocaine. He was placed on close observation checks meant to happen every 15 minutes. His family's lawsuit, filed in September 2025 in Franklin County against the state and contractor Wellpath, alleges staff last interacted with him at 8:57 p.m. on Sept. 9, 2023, and only observed him afterward; at 6:30 a.m. an officer found him cold and stiff, dead for at least four hours. The medical examiner ruled it an accidental fentanyl and hydroxyzine overdose. The Department of Corrections declined to comment. Its public death-investigation summaries stop at 2022, and the Defender General declined to share findings on Nichols' death. No ruling in the case has been reported.
Based on that information, the complaint states, Nichols was placed on “close observation checks” that were supposed to occur every 15 minutes.
The lawsuit against the state of Vermont and health care contractor Wellpath says officials failed to provide adequate care for Nichols, knowing he was going through drug withdrawals.
