What happened
A September 18, 2026 HHS Inspector General audit of Connecticut's Medicaid individualized home support program for people with intellectual and developmental disabilities found that, of 101 critical incidents such as ER-level injuries in state fiscal year 2024, 70 involving 61 people were not properly reported or recorded: providers never reported 48, reported 11 late, and the Department of Developmental Services failed to log 11 in its own system. DDS told auditors its claims-based monitoring system for catching unreported incidents was not fully in place during the audit period. At five provider site visits, auditors also found 29 missed or undocumented abuse-and-neglect registry checks and 23 missing criminal background checks; the state concurred with all four recommendations.
