EPA Office of Inspector General report 26-E-0025 (April 28, 2026) found that from 2020 through 2024 the EPA and some authorized states did not fully meet their commitment to inspect all large quantity generators of hazardous waste within the five-year period. Together they inspected 5,499, or 81 percent, of 6,827 such generators, and 4,661 inspections identified at least one violation. Of 53 states and territories analyzed, 38 committed to the standard plan of inspecting every generator at least once every five years; 30 reached at least 85 percent, five reached 71 to 84 percent and three reached 70 percent or below. Fifteen states followed approved alternative plans and on average inspected 65 percent of their generators.
