HHS Office of Inspector General audit A-02-24-01020, issued September 30, 2026, examined $505.5 million in school-based Medicaid services that Wisconsin claimed over a four-year period. OIG estimated that at least $67 million was improper because of unsupported random moment time study coding and services provided by unlicensed providers, and that a further $387.9 million may be improper because the state could not document the ratios used to allocate costs to Medicaid. OIG also said the time studies did not sample moments at the beginning of the school year. It made six recommendations, including refunds. The state agency generally disagreed with the findings and some recommendations and described steps it has taken to improve compliance.
