What happened
Utah's Office of the Legislative Auditor General audited the Division of Child and Family Services in a January 2026 report. It found caseworkers missed priority deadlines for face-to-face contact with children in 3,240 of 23,919 cases in fiscal year 2025, with the worst-performing offices missing deadlines in over 25 percent of cases, and that thousands of cases also had late safety assessments or lapses in supervisor oversight. The auditors attributed the variation to a lack of management control from division leadership down to caseworkers, and recommended performance targets and procedures for when staff fall short. The division's response plan is included in the report.
