The Justice Department Inspector General's May 21, 2026 inspection of the Bureau of Prisons' U.S. Penitentiary Canaan in Waymart, Pennsylvania (report 26-055) observed staff applying four-point restraints in an unsafe way that caused severe discomfort and risked lasting injury.
