City audit finds Indianapolis's Office of Public Health and Safety awarded contracts to vendors tied to its own staff and lacked basic financial oversight
An April 9, 2026 audit by Indianapolis's Office of Audit and Performance found the Office of Public Health and Safety (OPHS) had insufficient programmatic and invoice documentation, weak review of program effectiveness, and had awarded contracts to individuals and organizations with 'significant connections' to OPHS staff, among eight findings rated high or moderate risk. The city referred the matter to the Indiana State Board of Accounts, which opened its own review.
